No idea what version it was installed on but I've had it on several blogs although they were version 2.1 and up with no problems. It also could be another plugin you had was clashing with this. That does happen because plugins are individually developed sometimes they don't play nice with each other.I did get a tad worried because I also have subscribe to comments installed which I thought was the most likely plugin to clash with Comment Relish but I've been lucky.
